U pandemiji koronavirusa znanstvenike je uvijek zbunjivalo zbog čega su neki pacijenti zarazu preboljeli lako, gotovo i ne znajući da su inficirani, dok je kod drugih bolesnika došlo do teškog akutnog respiratornog sindroma, dugotrajnih post covid posljedica i smrti. Poznato je, naravno, da određeni komorbiditeti kao što su visoki tlak, dijabetes, ishemijske bolesti srčanih krvnih žila te pretilost djeluju kao ogroman rizični faktor za teže posljedice covid bolesti, no često se događa da potpuno zdrave osobe koje nemaju slabiji kardiovaskularni i respiratorni sustav završe s najgorim mogućim posljedicama.

U čemu je kvaka istražuje se u tisućama kliničkih studija, a najnovija, objavljena u časopisu Science Immunology sugerira kako su antitijela koja se okreću protiv vlastite imunološke obrane ključni pokretač teških bolesti i smrti nakon infekcije SARS-CoV-2 virusom kod nekih ljudi.

Ta 'odbjegla' antitijela, poznata kao autoantitijela, također su u malom dijelu prisutna u udjelu zdravih, nezaraženih osoba - a njihov postotak se povećava s godinama, što također objašnjava zašto su starije osobe izložene većem riziku od teškog COVID-19.

Ovo istraživanje na čijem je čelu bio imunolog Jean-Laurent Casanova sa Sveučilištu Rockefeller u New Yorku, otkriva da je oko 10 posto ljudi koji su imali tešku kliničku sliku, posjedovalo autoantitijela koja napadaju i blokiraju interferone tipa 1, molekule proteina u krvi koje imaju ključnu ulogu u borbi protiv virusnih infekcija.

Ovo nije prva bolest u kojoj imunološki sustav sam sabotira i zaustavi interferon da bi pravilno radio svoj posao. Neke bakterijske infekcije, poput onih uzrokovanih vrsta stafilokoka, često se povećavaju kada se proizvodnja antitijela u tijelu okrene protiv vlastite obrane interferona. Ovakve 'samosabotaže' zabilježene su i kod osoba liječenih interferonom zbog infekcije poput hepatitisa, kao i kod osoba koje boluju od autoimune bolesti lupus.

„Prvo veliko istraživanje o ovome objavljeno je prošle godine, i bilo je vjerojatno jedno od najvažnijih otkako je izbila pandemija. Ono što je napravljeno u novoj studiji jest da smo doista duboko istražili koliko su učestala ta autoantitijela generalno u populaciji i ispostavilo se da su zapanjujuće česta - objasnio je Aaron Ring, imunolog iz Škole za medicinu Sveučilišta Yale u New Havenu, u američkoj državi Connecticut.

Studiju su proveli na 3595 pacijenata iz 38 zemalja koji su imali vrlo teški oblik COVID-19, odnosno na osobama koje su završile na jedinicama intenzivne njege zbog težine bolesti.

Ukupno 13,6 posto pacijenata obuhvaćenih istraživanjem imalo je autoantitijela, od toga 9,6 posto pacijenata s tim antitijelima bilo je mlađe od 40 godina, a do 21 posto pacijenata u dobi iznad 80 godina. Autoantitijela su isto tako bila prisutna i u 18 posto pacijenata koji su preminuli od bolesti COVID-19.

Kako prenosi Scientific American dr. Casanova i njegove kolege sumnjali su da su ta zlonamjerna antitijela uzrok, a ne posljedica teške kliničke slike bolesti. Za to su postojale i dobre indicije, jer je njegov tim otkrio prisutnost tih antitijela u četiri od 1000 zdravih osoba čije su uzorke krvi prikupili prije izbijanja pandemije. Znanstveni tim otkrio je i da pojedinci s genetskim mutacijama koje ometaju aktivnost interferona tipa 1 imaju veći rizik od razvoja teškog oblika COVID-19.

Kako bi proučili tu poveznicu još više, istraživači su krenuli u potragu za autoantitijelima u masovnom broju krvnih uzoraka uzetima od gotovo 35.000 zdravih osoba prije pandemije. Kod njih 0,18 posto, u dobi između 18 i 69 godina, postoje autoantitijela koja napadaju i blokiraju interferone tipa 1, a omjer se nešto povećava s dobi. Antitijela su bila prisutna kod 1,1 posto zdravih pojedinaca u dobi između 70 i 79 godina te kod 3,4 posto zdravih pojedinaca u dobi iznad 80 godina.

Casanova dodaje da rezultati istraživanja imaju vrlo jasne kliničke implikacije te sugerira da bolnice provjeravaju kod covid pacijenta ta autoantitijela, kao i mutacije povezane s blokiranjem interferona tipa 1. To bi moglo identificirati ljude za koje postoji veća vjerojatnost da će se kritično razboljeti od COVID-19, što bi pomoglo liječnicima da na odgovarajući način prilagode svoje liječenje.

Znanstvenici tumače kako je uzorak od više od 30.000 ljudi prevelik da bi se zanemarilo njihovo otkriće.

"Ono pokazuje da je ovo nešto o čemu moramo razmisliti. A znanstvenici bi sada trebali razmotriti imaju li autoantitijela ulogu u pokretanju drugih zaraznih bolesti, kazao je imunolog Aaron Ring. Istraživački tim već je pronašao dokaze o autoantitijelima protiv različitih komponenti imunološkog sustava kod osoba oboljelih od COVID-19, a on i njegove kolege sada dodatno istražuju.

„Pretpostavljam da smo tek počeli grebati po površini", kazao je dr. Ring.

U pandemiji koronavirusa znanstvenike je uvijek zbunjivalo zbog čega su neki pacijenti zarazu preboljeli lako, gotovo i ne znajući da su inficirani, dok je kod drugih bolesnika došlo do teškog akutnog respiratornog sindroma, dugotrajnih post covid posljedica i smrti. Poznato je, naravno, da određeni komorbiditeti kao što su visoki tlak, dijabetes, ishemijske bolesti srčanih krvnih žila te pretilost djeluju kao ogroman rizični faktor za teže posljedice covid bolesti, no često se događa da potpuno zdrave osobe koje nemaju slabiji kardiovaskularni i respiratorni sustav završe s najgorim mogućim posljedicama.

U čemu je kvaka istražuje se u tisućama kliničkih studija, a najnovija, objavljena u časopisu Science Immunology sugerira kako su antitijela koja se okreću protiv vlastite imunološke obrane ključni pokretač teških bolesti i smrti nakon infekcije SARS-CoV-2 virusom kod nekih ljudi.

Ta 'odbjegla' antitijela, poznata kao autoantitijela, također su u malom dijelu prisutna u udjelu zdravih, nezaraženih osoba - a njihov postotak se povećava s godinama, što također objašnjava zašto su starije osobe izložene većem riziku od teškog COVID-19.

Ovo istraživanje na čijem je čelu bio imunolog Jean-Laurent Casanova sa Sveučilištu Rockefeller u New Yorku, otkriva da je oko 10 posto ljudi koji su imali tešku kliničku sliku, posjedovalo autoantitijela koja napadaju i blokiraju interferone tipa 1, molekule proteina u krvi koje imaju ključnu ulogu u borbi protiv virusnih infekcija.

Ovo nije prva bolest u kojoj imunološki sustav sam sabotira i zaustavi interferon da bi pravilno radio svoj posao. Neke bakterijske infekcije, poput onih uzrokovanih vrsta stafilokoka, često se povećavaju kada se proizvodnja antitijela u tijelu okrene protiv vlastite obrane interferona. Ovakve 'samosabotaže' zabilježene su i kod osoba liječenih interferonom zbog infekcije poput hepatitisa, kao i kod osoba koje boluju od autoimune bolesti lupus.

„Prvo veliko istraživanje o ovome objavljeno je prošle godine, i bilo je vjerojatno jedno od najvažnijih otkako je izbila pandemija. Ono što je napravljeno u novoj studiji jest da smo doista duboko istražili koliko su učestala ta autoantitijela generalno u populaciji i ispostavilo se da su zapanjujuće česta - objasnio je Aaron Ring, imunolog iz Škole za medicinu Sveučilišta Yale u New Havenu, u američkoj državi Connecticut.

Studiju su proveli na 3595 pacijenata iz 38 zemalja koji su imali vrlo teški oblik COVID-19, odnosno na osobama koje su završile na jedinicama intenzivne njege zbog težine bolesti.

Ukupno 13,6 posto pacijenata obuhvaćenih istraživanjem imalo je autoantitijela, od toga 9,6 posto pacijenata s tim antitijelima bilo je mlađe od 40 godina, a do 21 posto pacijenata u dobi iznad 80 godina. Autoantitijela su isto tako bila prisutna i u 18 posto pacijenata koji su preminuli od bolesti COVID-19.

Kako prenosi Scientific American dr. Casanova i njegove kolege sumnjali su da su ta zlonamjerna antitijela uzrok, a ne posljedica teške kliničke slike bolesti. Za to su postojale i dobre indicije, jer je njegov tim otkrio prisutnost tih antitijela u četiri od 1000 zdravih osoba čije su uzorke krvi prikupili prije izbijanja pandemije. Znanstveni tim otkrio je i da pojedinci s genetskim mutacijama koje ometaju aktivnost interferona tipa 1 imaju veći rizik od razvoja teškog oblika COVID-19.

Kako bi proučili tu poveznicu još više, istraživači su krenuli u potragu za autoantitijelima u masovnom broju krvnih uzoraka uzetima od gotovo 35.000 zdravih osoba prije pandemije. Kod njih 0,18 posto, u dobi između 18 i 69 godina, postoje autoantitijela koja napadaju i blokiraju interferone tipa 1, a omjer se nešto povećava s dobi. Antitijela su bila prisutna kod 1,1 posto zdravih pojedinaca u dobi između 70 i 79 godina te kod 3,4 posto zdravih pojedinaca u dobi iznad 80 godina.

Casanova dodaje da rezultati istraživanja imaju vrlo jasne kliničke implikacije te sugerira da bolnice provjeravaju kod covid pacijenta ta autoantitijela, kao i mutacije povezane s blokiranjem interferona tipa 1. To bi moglo identificirati ljude za koje postoji veća vjerojatnost da će se kritično razboljeti od COVID-19, što bi pomoglo liječnicima da na odgovarajući način prilagode svoje liječenje.

Znanstvenici tumače kako je uzorak od više od 30.000 ljudi prevelik da bi se zanemarilo njihovo otkriće.

"Ono pokazuje da je ovo nešto o čemu moramo razmisliti. A znanstvenici bi sada trebali razmotriti imaju li autoantitijela ulogu u pokretanju drugih zaraznih bolesti, kazao je imunolog Aaron Ring. Istraživački tim već je pronašao dokaze o autoantitijelima protiv različitih komponenti imunološkog sustava kod osoba oboljelih od COVID-19, a on i njegove kolege sada dodatno istražuju.

„Pretpostavljam da smo tek počeli grebati po površini", kazao je dr. Ring.
