Znanstvenici sa Sveučilišta u Oxfordu u zadnjem istraživanju, o kojem izvještava BBC, tvrde kako njihova studija pokazuje da su testovi na koronavirus preosjetljivi.

Preosjetljivi su, kaže studija, u toj mjeri da mogu otkriti tragove neaktivnih virusa od infekcije koja je prošla prije više tjedana, što onda može dovesti do lažno pozitivnog rezultata i uvećati broj inficiranih iako te osobe više nisu zarazne za okolinu.

Od početka pandemije vode se rasprave oko toga otkriva li pozitivan rezultat na testu aktivan virus, odnosno onaj koji se može umnožavati i širiti dalje, ili njegove fragmente koji se ne mogu dalje razmnožavati.

Kao što je poznato kad se provodi test na COVID-19, dobiva se odgovor je li uzorak pozitivan ili negativan. Pritom nema pokazatelja kolika je količina virusa u uzorku i u kolikoj je mjeri on aktivan. Tako i osoba koja u sebi ima veliku količinu virusa i ona kod koje su ostali samo njegovi fragmenti nakon preboljene infekcije dobiju jednako pozitivan rezultat testa.

Jedan od autora britanske studije, profesor Carl Heneghan, smatra da bi testovi na COVID-19 umjesto rezultata pozitivan odnosno negativan morali imati određeni prag koji ne bi pokazivao pozitivan rezultat ako je virus prisutan u vrlo maloj količini.

Prof. Heneghan kaže da bi se tako smanjio i broj osoba u strogoj izolaciji, kao i nepotrebno praćenje njihovih kontakata, a omogućilo bi se bolje razumijevanje trenutačnog stanja pandemije.

Igra detekcije

Centar za primijenjenu medicinu Sveučilišta u Oxfordu ispitao je nalaze 25 studija u kojima je analizirano hoće li se uzorci virusa razviti u staklenoj posudi u laboratoriju. Došli su do zaključka da je većina ljudi zarazna samo tjedan dana, ali da testovi mogu pokazivati pozitivan rezultat na COVID i tjednima nakon toga.

Kako doznajemo i u Hrvatskoj se razmatra mogućnosti da osobama koje nemaju simptome bolesti, a imaju pozitivan test, bude nešto skraćena izolacija, dok bi onima sa simptomima i dalje ostala dva tjedna. No, službena odluka u tom smjeru još nije donesena. Hrvatsko epidemiološko društvo pak predlaže da osobe zaražene koronavirusom idu u 14-dnevnu samoizolaciju samo ako rade u zdravstvu i sustavu socijalne skrbi.

Mikrobiolozi objašnjavaju kako PCR testovi detektiraju genetski materijal iz virusa u ciklusima: što je manje ciklusa potrebno za detekciju, veća je količina virusa u uzorku. Što je veća količina virusa, vjerojatnije je da će pacijent biti zarazan. Podatak o broju ciklusa potrebnih da se virus pronađe, tzv. prag ciklusa, nije uključen u rezultate koji se šalju pacijentu.

- Kalibriranje PCR testova, odnosno broj ciklusa na kojemu će se odrediti granica pozitivnog u odnosnu na negativni nalaz stvar je znanstvene rasprave i moguće je da će u nekom trenutku doći do promjene od sadašnjih preporuka - kaže nam epidemiolog prof. dr. Branko Kolarić.

Objašnjava kako sve one osobe koje nose virus u sebi nazivamo zaraženima.

Dan po dan

- Neke imaju simptome bolesti kao što su temperatura, kašalj, gubitak okusa i mirisa ili glavobolja, dok drugi nemaju te simptome i zato ih nazivamo asimptomatskim slučajevima. Oni su manje zarazni za druge jer u sebi imaju manje virusa nego oni koji su razvili simptome pa kišu i virus dalje šire. Međutim, i oni virus mogu izlučivati 14 dana i upravo zato mi testiramo kontakte oboljelih ili ih šaljemo u samoizolaciju. Iako su bez simptoma, mogu zaraziti druge koji možda neće reagirati tako blago kao oni - napominje Kolarić.

Što se tiče RT-PCR testova, oni su tako napravljeni da budu maksimalno osjetljivi, odnosno da je mogućnost lažno negativnog nalaza što bliža nuli.

- Točno je da taj test detektira i ostatke genetskog materijala virusa, kada se virus više aktivno ne razmnožava, odnosno ne izlučuje kao infektivna čestica u okoliš.

Upravo je to razlog zašto u preporukama nemamo više testiranje nakon što prođe minimalno 14 dana od početka simptoma, uz uvjet da su se simptomi povukli najmanje tri zadnja dana. Osobu s pozitivnim nalazom PCR testa koja najmanje zadnja tri dana nema simptome, a prošlo je dva tjedna od prvog simptoma, ne smatramo zaraznom za druge osobe - navodi dr. Kolarić.

Epidemiolozi predlažu 14-dnevnu karantenu samo za određene skupine


Hrvatsko epidemiološko društvo poslalo je ministru zdravstva Viliju Berošu pismo u kojem predlažu promjenu mjera samoizolacije. Oni predlažu da osobe zaražene koronavirusom idu u 14-dnevnu samoizolaciju samo ako rade u zdravstvu i sustavu socijalne skrbi. 

"S obzirom na poznatu činjenicu da je SARS-CoV-2 virus u lokalnoj transmisiji, te potrebu pridržavanja epidemioloških mjera (fizička distanca, nošenje maski, higijena i dezinfekcija) od strane svih, potrebno je razmisliti o promjeni strategije koja uključuje stavljanje u samoizolaciju zaražene osobe (pozitivne na SARS—CoV-2 virus) i kontakte zdravstvene djelatnike, te one koji rade u domovima za starije i nemoćne osobe.

Svi ostali ustanovljeni kontakti moraju se ponašati osobno odgovorno u skladu s već spomenutim epidemiološkim mjerama - navodi se u pismu.

Znanstvenici sa Sveučilišta u Oxfordu u zadnjem istraživanju, o kojem izvještava BBC, tvrde kako njihova studija pokazuje da su testovi na koronavirus preosjetljivi.

Preosjetljivi su, kaže studija, u toj mjeri da mogu otkriti tragove neaktivnih virusa od infekcije koja je prošla prije više tjedana, što onda može dovesti do lažno pozitivnog rezultata i uvećati broj inficiranih iako te osobe više nisu zarazne za okolinu.

Od početka pandemije vode se rasprave oko toga otkriva li pozitivan rezultat na testu aktivan virus, odnosno onaj koji se može umnožavati i širiti dalje, ili njegove fragmente koji se ne mogu dalje razmnožavati.

Kao što je poznato kad se provodi test na COVID-19, dobiva se odgovor je li uzorak pozitivan ili negativan. Pritom nema pokazatelja kolika je količina virusa u uzorku i u kolikoj je mjeri on aktivan. Tako i osoba koja u sebi ima veliku količinu virusa i ona kod koje su ostali samo njegovi fragmenti nakon preboljene infekcije dobiju jednako pozitivan rezultat testa.

Jedan od autora britanske studije, profesor Carl Heneghan, smatra da bi testovi na COVID-19 umjesto rezultata pozitivan odnosno negativan morali imati određeni prag koji ne bi pokazivao pozitivan rezultat ako je virus prisutan u vrlo maloj količini.

Prof. Heneghan kaže da bi se tako smanjio i broj osoba u strogoj izolaciji, kao i nepotrebno praćenje njihovih kontakata, a omogućilo bi se bolje razumijevanje trenutačnog stanja pandemije.

Igra detekcije

Centar za primijenjenu medicinu Sveučilišta u Oxfordu ispitao je nalaze 25 studija u kojima je analizirano hoće li se uzorci virusa razviti u staklenoj posudi u laboratoriju. Došli su do zaključka da je većina ljudi zarazna samo tjedan dana, ali da testovi mogu pokazivati pozitivan rezultat na COVID i tjednima nakon toga.

Kako doznajemo i u Hrvatskoj se razmatra mogućnosti da osobama koje nemaju simptome bolesti, a imaju pozitivan test, bude nešto skraćena izolacija, dok bi onima sa simptomima i dalje ostala dva tjedna. No, službena odluka u tom smjeru još nije donesena. Hrvatsko epidemiološko društvo pak predlaže da osobe zaražene koronavirusom idu u 14-dnevnu samoizolaciju samo ako rade u zdravstvu i sustavu socijalne skrbi.

Mikrobiolozi objašnjavaju kako PCR testovi detektiraju genetski materijal iz virusa u ciklusima: što je manje ciklusa potrebno za detekciju, veća je količina virusa u uzorku. Što je veća količina virusa, vjerojatnije je da će pacijent biti zarazan. Podatak o broju ciklusa potrebnih da se virus pronađe, tzv. prag ciklusa, nije uključen u rezultate koji se šalju pacijentu.

- Kalibriranje PCR testova, odnosno broj ciklusa na kojemu će se odrediti granica pozitivnog u odnosnu na negativni nalaz stvar je znanstvene rasprave i moguće je da će u nekom trenutku doći do promjene od sadašnjih preporuka - kaže nam epidemiolog prof. dr. Branko Kolarić.

Objašnjava kako sve one osobe koje nose virus u sebi nazivamo zaraženima.

Dan po dan

- Neke imaju simptome bolesti kao što su temperatura, kašalj, gubitak okusa i mirisa ili glavobolja, dok drugi nemaju te simptome i zato ih nazivamo asimptomatskim slučajevima. Oni su manje zarazni za druge jer u sebi imaju manje virusa nego oni koji su razvili simptome pa kišu i virus dalje šire. Međutim, i oni virus mogu izlučivati 14 dana i upravo zato mi testiramo kontakte oboljelih ili ih šaljemo u samoizolaciju. Iako su bez simptoma, mogu zaraziti druge koji možda neće reagirati tako blago kao oni - napominje Kolarić.

Što se tiče RT-PCR testova, oni su tako napravljeni da budu maksimalno osjetljivi, odnosno da je mogućnost lažno negativnog nalaza što bliža nuli.

- Točno je da taj test detektira i ostatke genetskog materijala virusa, kada se virus više aktivno ne razmnožava, odnosno ne izlučuje kao infektivna čestica u okoliš.

Upravo je to razlog zašto u preporukama nemamo više testiranje nakon što prođe minimalno 14 dana od početka simptoma, uz uvjet da su se simptomi povukli najmanje tri zadnja dana. Osobu s pozitivnim nalazom PCR testa koja najmanje zadnja tri dana nema simptome, a prošlo je dva tjedna od prvog simptoma, ne smatramo zaraznom za druge osobe - navodi dr. Kolarić.

Epidemiolozi predlažu 14-dnevnu karantenu samo za određene skupine


Hrvatsko epidemiološko društvo poslalo je ministru zdravstva Viliju Berošu pismo u kojem predlažu promjenu mjera samoizolacije. Oni predlažu da osobe zaražene koronavirusom idu u 14-dnevnu samoizolaciju samo ako rade u zdravstvu i sustavu socijalne skrbi. 

"S obzirom na poznatu činjenicu da je SARS-CoV-2 virus u lokalnoj transmisiji, te potrebu pridržavanja epidemioloških mjera (fizička distanca, nošenje maski, higijena i dezinfekcija) od strane svih, potrebno je razmisliti o promjeni strategije koja uključuje stavljanje u samoizolaciju zaražene osobe (pozitivne na SARS—CoV-2 virus) i kontakte zdravstvene djelatnike, te one koji rade u domovima za starije i nemoćne osobe.

Svi ostali ustanovljeni kontakti moraju se ponašati osobno odgovorno u skladu s već spomenutim epidemiološkim mjerama - navodi se u pismu.
