Sepsa je vrlo vjerojatno jedan od najvećih, ako ne i najveći protivnik moderne medicine.

Prvi podaci o sepsi su zabilježeni još 3000 godina prije nove ere, a prema podacima koje je Svjetska zdravstvena organizacija objavila 2020. godine, u svijetu je 2017. godine od sepse oboljelo 49 milijuna ljudi.

– Mortalitet je bio oko 11 milijuna ljudi, dakle, veći od 20 posto. Inače, mortalitet od sepse, odnosno od septičkog šoka u razvijenim zemljama zapadnog svijeta je 38 posto (EU, Sjeverna Amerika, Australija). Sada zamislite koliki je mortalitet u nerazvijenim državama – kazuje nam prof. dr. sc. Nenad Karanović, poznati splitski anesteziolog i bivši predstojnik Klinike za anesteziologiju i intenzivno liječenje (JIL) KBC-a Split.

Kako je nedavno dio medija objavio, u Hrvatskoj još uvijek ne postoje smjernice za prevenciju i liječenje sepse.

– Istina je da još uvijek ne postoje hrvatske smjernice, međutim, treba istaknuti da mi u Hrvatskoj radimo po svjetskim smjernicama liječenja sepse koje se stalno mijenjaju, svakih nekoliko godina kako se mijenja znanje, odnosno nova saznanja o sepsi. Zadnji put su svjetske smjernice mijenjane 2021. godine – kaže dr. Karanović.

Što je ustvari sepsa?

– Sepsa je životno ugrožujuća disfunkcija organa uzrokovana poremećenim odgovorom domaćina (čovjeka ili životinje) na infekciju. Sepsa se ne dobiva samo u bolnicama, kako naši ljudi pojednostavljeno misle, ona se može dobiti bilo gdje, a najčešće je vezana uz urinarni trakt. Danas je sepsa jako veliki javnozdravstveni problem.

Gdje pacijenti u bolnicama najlakše "peškaju" sepsu?

– Sepsu se može dobiti na svim odjelima, nije ona vezana za neki odjel posebno. Međutim, JIL je mjesto gdje se nekako najlakše može dobiti. Ne zbog toga jer je tamo prljavo, što nije istina, ne zbog toga jer tamo rade loši liječnici, u pravilu tamo rade vrhunski liječnici, već zbog toga jer u JIL dolaze pacijenti u jako teškom stanju, s lošim imunitetom, nad kojima provodimo monitoring. U njihovom liječenju uključeni su razni kateteri, respiratori i drugi uređaji i vrlo je lako moguće da se ono što je u čovjeku lako aktivira. Ponavljam radi se o ljudima s jako oštećenim imunitetom.

Iako jedno od najsmrtonosnijih oboljenja, sepsa još uvijek nije dovoljno istražena?

– Nažalost, ta ležeća patofiziologija nije poznata u potpunosti. Sepsa može zahvatiti od mozga, srca, pluća, bubrega, jetara... i širi se krvlju i limfom. Razlozi zbog čega je netko dobio sepsu mogu biti genetski, veliku ulogu igra životna dob oboljele osobe, komorbiditeti (prateće bolesti) koje sa sobom nosi, slabost organizma, iscrpljenost i vrsta i količina patogena i njihova virulencija, kao i mjesto infekcije.

U liječenju sepse važnu ulogu igra vrijeme, odnosno period unutar kojega se detektira sepsa.

– Kada nastane ta imunosupresija bitno je pronaći sjelo sepse. Kada sepsa napadne urotrakt smrtnost je u prosjeku 26 posto, kod pluća smrtnost je od 50 do 55 posto, a kada dođe do propadanja crijeva tada je smrtnost 78 posto. Najbolje bi bilo unutar jednog sata od detektiranja sepse dati pacijentu antimikrobik, no, nažalost, to nikada ili gotovo nikada nije unutar jednog sata. Zašto? Pa iz jednostavnog razloga jer za točno utvrđivanje sepse treba najmanje od dva do tri sata – ističe dr. Karanović.

Oboljeli od sepse imaju gotovo uvijek problema s disanjem?

– Tada im dajemo kisik ili ih stavljamo na strojnu ventilaciju. Nakon toga dajemo antimikrobik empirijski i razne tekućine. Daljnju terapiju dajemo po potrebi, a ujedno oboljelima podržavamo srčanu funkciju i tlak. Preživljavanje naravno ovisi i o izdržljivosti samog organizma. Moram istaknuti da smo u KBC-u Split imali jako dobre rezultate preživljavanja, usporedive s europskim bolnicama.

Sepsu mogu izazvati bakterije, gljive, paraziti i virusi...

– Sepsu dijagnosticiramo na temelju kliničke slike, radiološke i laboratorijske dijagnostike unutar koje je jako značajna mikrobiološka dijagnostika. Liječenje sepse je jako složeno i nakon preboljenja kod izliječenih osoba​ veći je rizik od mortaliteta, jer je sama sepsa oštetila vitalne organe – zaključuje prof. dr. sc. Nenad Karanović.

Problem s antibioticima

– U JIL-ovima diljem svijeta se vrlo često javljaju infekcije uzrokovane multiplo rezistentnim mikroorganizmima. Problem je u antibiotskom liječenju jer antibiotici ne djeluju na njih i onda se uključuju rezervni antibiotici kojih na raspolaganju ima sve manje – kazuje dr. Karanović.

Sepsa je vrlo vjerojatno jedan od najvećih, ako ne i najveći protivnik moderne medicine.

Prvi podaci o sepsi su zabilježeni još 3000 godina prije nove ere, a prema podacima koje je Svjetska zdravstvena organizacija objavila 2020. godine, u svijetu je 2017. godine od sepse oboljelo 49 milijuna ljudi.

– Mortalitet je bio oko 11 milijuna ljudi, dakle, veći od 20 posto. Inače, mortalitet od sepse, odnosno od septičkog šoka u razvijenim zemljama zapadnog svijeta je 38 posto (EU, Sjeverna Amerika, Australija). Sada zamislite koliki je mortalitet u nerazvijenim državama – kazuje nam prof. dr. sc. Nenad Karanović, poznati splitski anesteziolog i bivši predstojnik Klinike za anesteziologiju i intenzivno liječenje (JIL) KBC-a Split.

Kako je nedavno dio medija objavio, u Hrvatskoj još uvijek ne postoje smjernice za prevenciju i liječenje sepse.

– Istina je da još uvijek ne postoje hrvatske smjernice, međutim, treba istaknuti da mi u Hrvatskoj radimo po svjetskim smjernicama liječenja sepse koje se stalno mijenjaju, svakih nekoliko godina kako se mijenja znanje, odnosno nova saznanja o sepsi. Zadnji put su svjetske smjernice mijenjane 2021. godine – kaže dr. Karanović.

Što je ustvari sepsa?

– Sepsa je životno ugrožujuća disfunkcija organa uzrokovana poremećenim odgovorom domaćina (čovjeka ili životinje) na infekciju. Sepsa se ne dobiva samo u bolnicama, kako naši ljudi pojednostavljeno misle, ona se može dobiti bilo gdje, a najčešće je vezana uz urinarni trakt. Danas je sepsa jako veliki javnozdravstveni problem.

Gdje pacijenti u bolnicama najlakše "peškaju" sepsu?

– Sepsu se može dobiti na svim odjelima, nije ona vezana za neki odjel posebno. Međutim, JIL je mjesto gdje se nekako najlakše može dobiti. Ne zbog toga jer je tamo prljavo, što nije istina, ne zbog toga jer tamo rade loši liječnici, u pravilu tamo rade vrhunski liječnici, već zbog toga jer u JIL dolaze pacijenti u jako teškom stanju, s lošim imunitetom, nad kojima provodimo monitoring. U njihovom liječenju uključeni su razni kateteri, respiratori i drugi uređaji i vrlo je lako moguće da se ono što je u čovjeku lako aktivira. Ponavljam radi se o ljudima s jako oštećenim imunitetom.

Iako jedno od najsmrtonosnijih oboljenja, sepsa još uvijek nije dovoljno istražena?

– Nažalost, ta ležeća patofiziologija nije poznata u potpunosti. Sepsa može zahvatiti od mozga, srca, pluća, bubrega, jetara... i širi se krvlju i limfom. Razlozi zbog čega je netko dobio sepsu mogu biti genetski, veliku ulogu igra životna dob oboljele osobe, komorbiditeti (prateće bolesti) koje sa sobom nosi, slabost organizma, iscrpljenost i vrsta i količina patogena i njihova virulencija, kao i mjesto infekcije.

U liječenju sepse važnu ulogu igra vrijeme, odnosno period unutar kojega se detektira sepsa.

– Kada nastane ta imunosupresija bitno je pronaći sjelo sepse. Kada sepsa napadne urotrakt smrtnost je u prosjeku 26 posto, kod pluća smrtnost je od 50 do 55 posto, a kada dođe do propadanja crijeva tada je smrtnost 78 posto. Najbolje bi bilo unutar jednog sata od detektiranja sepse dati pacijentu antimikrobik, no, nažalost, to nikada ili gotovo nikada nije unutar jednog sata. Zašto? Pa iz jednostavnog razloga jer za točno utvrđivanje sepse treba najmanje od dva do tri sata – ističe dr. Karanović.

Oboljeli od sepse imaju gotovo uvijek problema s disanjem?

– Tada im dajemo kisik ili ih stavljamo na strojnu ventilaciju. Nakon toga dajemo antimikrobik empirijski i razne tekućine. Daljnju terapiju dajemo po potrebi, a ujedno oboljelima podržavamo srčanu funkciju i tlak. Preživljavanje naravno ovisi i o izdržljivosti samog organizma. Moram istaknuti da smo u KBC-u Split imali jako dobre rezultate preživljavanja, usporedive s europskim bolnicama.

Sepsu mogu izazvati bakterije, gljive, paraziti i virusi...

– Sepsu dijagnosticiramo na temelju kliničke slike, radiološke i laboratorijske dijagnostike unutar koje je jako značajna mikrobiološka dijagnostika. Liječenje sepse je jako složeno i nakon preboljenja kod izliječenih osoba​ veći je rizik od mortaliteta, jer je sama sepsa oštetila vitalne organe – zaključuje prof. dr. sc. Nenad Karanović.

Problem s antibioticima

– U JIL-ovima diljem svijeta se vrlo često javljaju infekcije uzrokovane multiplo rezistentnim mikroorganizmima. Problem je u antibiotskom liječenju jer antibiotici ne djeluju na njih i onda se uključuju rezervni antibiotici kojih na raspolaganju ima sve manje – kazuje dr. Karanović.
