Čak i u vrijeme pandemije covida-19, tuberkuloza (TB) ostaje jedna od najsmrtonosnijih zaraznih bolesti na svijetu, upozorili su Svjetska zdravstvena organizacija (WHO) i Europski centar za prevenciju i kontrolu bolesti (ECDC) u četvrtak, 24. ožujka, kada se obilježava Svjetski dan borbe protiv tuberkuloze.

Pad registriranih slučajeva tuberkuloze od 24 posto u razdoblju između 2019. i 2020. vjerojatno je posljedica pandemije covida koja je utjecala na dijagnosticiranje i prijavu novih slučajeva te bolesti, no tuberkuloza je u ovom trenutku druga najsmrtonosnija zarazna bolest iza covida-19, objavile su dvije zdravstvene organizacije.

ECDC i WHO su upozorili da je od iznimne važnosti ulagati u borbu protiv tuberkuloze.

Premda su europske države i zemlje središnje Azije za 20 posto premašile cilj, kada je posrijedi smanjenje broja slučajeva tuberkuloze u roku od pet godina, nema mjesta za zadovoljstvo, smatra Hans Kluge, regionalni direktor WHO-a.

Da bismo postigli napredak u borbi protiv te zarazne bakterijske bolesti, morali bismo uvesti inovativne i još učinkovitije načine dijagnosticiranja i liječenja, a trebalo bi uvesti i više novih lijekova otpornih na tu bolest, istaknuo je Kluge.

Direktorica ECDC-a Andrea Ammon je dodala da je, usred pandemije covid-19, dakle, u posljednje dvije godine "borba protiv tuberkuloze, nažalost, postala još teža".

Na području Europe, koja prema standardima WHO-a obuhvaća 53 zemlje, u 2020. godini je prijavljeno više od 160.000 novih slučajeva tuberkuloze, a prethodne godine ih je bilo 216.000.

Pet stvari bi trebalo znati o ovome bacilu koji prati čovjeka tisućama godina.

Vrlo staro porijeklo

Današnja porodica bacila tuberkuloze se, prema paleogenetici pojavila prije otprilike 40.000 godina. Paleogenetika je disciplina koja nam omogućuje da razaberemo genetsku evoluciju organizama tijekom stoljeća.

Dugo se vjerovalo da je tuberkuloza u ljudi proizašla iz tuberkuloze goveda i da je na ljude prešla pojavom stočarstva još u neolitiku.

No nedavne su studije pokazale da je ova zarazna bolest postojala među ljudima i prije nego što su pripitomili stoku. O tomu svjedoče tragovi tuberkuloze pronađeni na ljudskim ostacima starim 11.000 godina.

Cjepivo staro sto godina

Nijemac Robert Koch je 1882. godine otkrio bacil odgovoran za razvoj tuberkuloze te je pokušao razviti serum za liječenje bolesti, no bez uspjeha. 

Prvo učinkovito cjepivo, BCG, otkrila su tek 1921. godine dvojica znanstvenika s Instituta Pasteur, Albert Calmette i Camille Guérin.

Stoljeće kasnije, cjepivo se i dalje koristi i još je uvijek vrlo učinkovito u zaštiti djece, no nije toliko učinkovito kod nekih oblika tuberkuloze koji pogađaju odrasle.

U Francuskoj BCG cjepivo nije obvezno od 2007. godine, ali se "preporučuje" za djecu koja su "izložena visokom riziku od razvoja tuberkuloze" zbog okolnosti u kojima žive ili zbog mjesta stanovanja.

Manja učinkovitost antibiotika

Nakon Drugoga svjetskog rata, otkriće streptomicina, a potom još nekoliko antibiotika, omogućuje učinkovitu borbu protiv plućne tuberkuloze, koja se osobito raširila među odraslima. 

No s vremenom se pojavljuju i sojevi Kochova bacila, otporni na spomenute lijekove, što liječnike primorava da propisuju 'koktele' antibiotika da bi učinkovito uništili bakteriju, a liječenje traje i po nekoliko mjeseci.

No na kraju dolazi do višestruke bakterijske rezistencije na lijekove, nerijetko zbog loše doziranih ili nedovoljno dugih tretmana liječenja antibioticima, što rezultira neuspjelim izlječenjem bolesti.  

Druga najsmrtonosnija zarazna bolest iza covida-19

Po posljednjim dostupnim podacima WHO-a, u 2020. godini tuberkuloza je bila drugi vodeći uzrok smrti od zaraznih bolesti, s 1,5 milijuna smrtnih ishoda. 

Do danas je zaraza tuberkulozom najzastupljenija u ljudskoj populaciji i prednjači nad AIDS-om, od kojega je u 2020. umrlo 680.000 ljudi, podaci su UNAIDS-a.

S trona ju je svrgnula najnovija zarazna bolest, covid-19, koji je u 2020. ubio više od 1,8 milijuna ljudi.

Treba podsjetiti da je tuberkuloza glavni uzrok smrti među HIV pozitivnim osobama.

Kao posljedica globalne neorganiziranosti povezane s pandemijom covida-19 (manje probira i liječenja), broj smrtnih slučajeva od tuberkuloze ponovno je počeo rasti i u 2020. je taj porast iznosio 7 posto. 

Na pet kontinenata

Tuberkuloza je prisutna na pet kontinenata, ali posebno pogađa zemlje u razvoju. Po podacima WHO-a, u 2020. je najveći broj novih slučajeva evidentiran u jugoistočnoj Aziji - 43 posto novih slučajeva i u Africi, gdje je zabilježeno 25 posto novih slučajeva. 

Dvije trećine slučajeva koncentrirano je u osam zemalja: Indiji, Kini, Indoneziji, Filipinima, Pakistanu, Nigeriji, Bangladešu i Južnoafričkoj Republici.

Čak i u vrijeme pandemije covida-19, tuberkuloza (TB) ostaje jedna od najsmrtonosnijih zaraznih bolesti na svijetu, upozorili su Svjetska zdravstvena organizacija (WHO) i Europski centar za prevenciju i kontrolu bolesti (ECDC) u četvrtak, 24. ožujka, kada se obilježava Svjetski dan borbe protiv tuberkuloze.

Pad registriranih slučajeva tuberkuloze od 24 posto u razdoblju između 2019. i 2020. vjerojatno je posljedica pandemije covida koja je utjecala na dijagnosticiranje i prijavu novih slučajeva te bolesti, no tuberkuloza je u ovom trenutku druga najsmrtonosnija zarazna bolest iza covida-19, objavile su dvije zdravstvene organizacije.

ECDC i WHO su upozorili da je od iznimne važnosti ulagati u borbu protiv tuberkuloze.

Premda su europske države i zemlje središnje Azije za 20 posto premašile cilj, kada je posrijedi smanjenje broja slučajeva tuberkuloze u roku od pet godina, nema mjesta za zadovoljstvo, smatra Hans Kluge, regionalni direktor WHO-a.

Da bismo postigli napredak u borbi protiv te zarazne bakterijske bolesti, morali bismo uvesti inovativne i još učinkovitije načine dijagnosticiranja i liječenja, a trebalo bi uvesti i više novih lijekova otpornih na tu bolest, istaknuo je Kluge.

Direktorica ECDC-a Andrea Ammon je dodala da je, usred pandemije covid-19, dakle, u posljednje dvije godine "borba protiv tuberkuloze, nažalost, postala još teža".

Na području Europe, koja prema standardima WHO-a obuhvaća 53 zemlje, u 2020. godini je prijavljeno više od 160.000 novih slučajeva tuberkuloze, a prethodne godine ih je bilo 216.000.

Pet stvari bi trebalo znati o ovome bacilu koji prati čovjeka tisućama godina.

Vrlo staro porijeklo

Današnja porodica bacila tuberkuloze se, prema paleogenetici pojavila prije otprilike 40.000 godina. Paleogenetika je disciplina koja nam omogućuje da razaberemo genetsku evoluciju organizama tijekom stoljeća.

Dugo se vjerovalo da je tuberkuloza u ljudi proizašla iz tuberkuloze goveda i da je na ljude prešla pojavom stočarstva još u neolitiku.

No nedavne su studije pokazale da je ova zarazna bolest postojala među ljudima i prije nego što su pripitomili stoku. O tomu svjedoče tragovi tuberkuloze pronađeni na ljudskim ostacima starim 11.000 godina.

Cjepivo staro sto godina

Nijemac Robert Koch je 1882. godine otkrio bacil odgovoran za razvoj tuberkuloze te je pokušao razviti serum za liječenje bolesti, no bez uspjeha. 

Prvo učinkovito cjepivo, BCG, otkrila su tek 1921. godine dvojica znanstvenika s Instituta Pasteur, Albert Calmette i Camille Guérin.

Stoljeće kasnije, cjepivo se i dalje koristi i još je uvijek vrlo učinkovito u zaštiti djece, no nije toliko učinkovito kod nekih oblika tuberkuloze koji pogađaju odrasle.

U Francuskoj BCG cjepivo nije obvezno od 2007. godine, ali se "preporučuje" za djecu koja su "izložena visokom riziku od razvoja tuberkuloze" zbog okolnosti u kojima žive ili zbog mjesta stanovanja.

Manja učinkovitost antibiotika

Nakon Drugoga svjetskog rata, otkriće streptomicina, a potom još nekoliko antibiotika, omogućuje učinkovitu borbu protiv plućne tuberkuloze, koja se osobito raširila među odraslima. 

No s vremenom se pojavljuju i sojevi Kochova bacila, otporni na spomenute lijekove, što liječnike primorava da propisuju 'koktele' antibiotika da bi učinkovito uništili bakteriju, a liječenje traje i po nekoliko mjeseci.

No na kraju dolazi do višestruke bakterijske rezistencije na lijekove, nerijetko zbog loše doziranih ili nedovoljno dugih tretmana liječenja antibioticima, što rezultira neuspjelim izlječenjem bolesti.  

Druga najsmrtonosnija zarazna bolest iza covida-19

Po posljednjim dostupnim podacima WHO-a, u 2020. godini tuberkuloza je bila drugi vodeći uzrok smrti od zaraznih bolesti, s 1,5 milijuna smrtnih ishoda. 

Do danas je zaraza tuberkulozom najzastupljenija u ljudskoj populaciji i prednjači nad AIDS-om, od kojega je u 2020. umrlo 680.000 ljudi, podaci su UNAIDS-a.

S trona ju je svrgnula najnovija zarazna bolest, covid-19, koji je u 2020. ubio više od 1,8 milijuna ljudi.

Treba podsjetiti da je tuberkuloza glavni uzrok smrti među HIV pozitivnim osobama.

Kao posljedica globalne neorganiziranosti povezane s pandemijom covida-19 (manje probira i liječenja), broj smrtnih slučajeva od tuberkuloze ponovno je počeo rasti i u 2020. je taj porast iznosio 7 posto. 

Na pet kontinenata

Tuberkuloza je prisutna na pet kontinenata, ali posebno pogađa zemlje u razvoju. Po podacima WHO-a, u 2020. je najveći broj novih slučajeva evidentiran u jugoistočnoj Aziji - 43 posto novih slučajeva i u Africi, gdje je zabilježeno 25 posto novih slučajeva. 

Dvije trećine slučajeva koncentrirano je u osam zemalja: Indiji, Kini, Indoneziji, Filipinima, Pakistanu, Nigeriji, Bangladešu i Južnoafričkoj Republici.

Čak i u vrijeme pandemije covida-19, tuberkuloza (TB) ostaje jedna od najsmrtonosnijih zaraznih bolesti na svijetu, upozorili su Svjetska zdravstvena organizacija (WHO) i Europski centar za prevenciju i kontrolu bolesti (ECDC) u četvrtak, 24. ožujka, kada se obilježava Svjetski dan borbe protiv tuberkuloze.
