Doktor Đivo Ljubičić, pulmolog u Kliničkoj bolnici Dubrava, ukazao je na pogreške u tekstu objavljenom u srijedu na portalu Slobodne Dalmacije.

Sporni tekst prenosi savjete liječnika iz Srbije za oporavak od Covida, koji između ostalog uključuju uzimanje acetilsalicilne kiseline nekoliko mjeseci nakon preboljenja.

Budući da je izrazito zaposlen dežurstvima u Covid intenzivnoj njezi, doktor Ljubičić nije imao vremena za telefonski razgovor, ali nam je proslijedio najnovije Smjernice za liječenje oboljelih od Covida koje propisuje Hrvatski zavod za javno zdravstvo, kako bismo javnost izvijestili o trenutno dostupnim lijekovima za borbu protiv ove bolesti.

Smjernice su namijenjene kliničarima, u prvom redu infektolozima i intenzivistima, ali i drugim zdravstvenim djelatnicima koji sudjeluju u liječenju bolesnika s Covidom-19.

O primjeni se odlučuje ovisno o težini bolesti

Na samom početku navodi se kako trenutno nema dovoljno dokaza o učinkovitim i sigurnim lijekovima ili kombinaciji lijekova za liječenje Covida. To je važno osvijestiti s obzirom na postojanje niza nepouzdanih internetskih izvora koji tvrde drugačije te svojim čitateljima ili pratiteljima preporučuju raznorazne neprovjerene supstance.

U tekstu se ne navode neke od popularnih opcija, poput ivermektina ili spomenute acetilsalicilne kiseline, ali su nabrojani preporučeni lijekovi, kao i oni koje treba izbjegavati. Budući da su smjernice namijenjene medicinskim stručnjacima, sve lijekove i metode opisane u dokumentu nije potrebni prenositi. U nastavku je skraćen pregled preporučenih i odobrenih lijekova koje propisuju, i najvećim dijelom primjenjuju, zdravstveni stručnjaci.

Osnova liječenja je ublažavanje simptoma te sprječavanje komplikacija i napredovanja bolesti, uz mjere intenzivnog liječenja za teže oboljele.

Prema Smjernicama, o vrsti liječenja odlučuje kliničar na temelju procjene težine konkretnog slučaja oboljenja. 

Različiti su stupnjevi bolesti, pa se tako ona kreće od asimptomatskih infekcija, preko blagog, srednje teškog i teškog oblika, sve do kritičnog oblika koji može uključivati sepsu, zatajenje organa ili poremećaj svijesti. Teži oblik bolesti koji se manifestira kod dijela oboljelih uključuje i akutni respiratorni distres sindrom (ARDS), jednu vrstu respiratornog zatajenja koja dovodi do nakupljanja tekućine u plućima, a počinje sa zaduhom i plitkim disanjem, a zbog niske razine kisika u krvi rad srca i mozga može zakazati.

Dostupni lijekovi i preporučene doze

U Smjernicama su propisani dozvoljeni lijekovi i način njihove primjene, a neke od njih navodimo u nastavku. Njihovo korištenje ne preporuča se izvan bolnice, budući da je tijekom primjene neophodan intenzivan nadzor njihove učinkovitosti i sigurnosti, što se može postići samo u hospitaliziranih bolesnika.

Remdesivir se trenutno preporuča za bolesnike s težom pneumonijom koji imaju urednu bubrežnu funkciju. Ako su ostvareni svi preduvjeti, liječenje treba započeti što je prije moguće. Primjena nakon petnaestog dana u većini slučajeva nije opravdana. Ovaj se lijek primjenjuje u polusatnoj intravenoznoj infuziji, a kod težih se slučajeva liječenje remdesivirom provodi tijekom deset dana.

Kortikosteroidi su se pokazali potencijalno korisnima u liječenju teškog i kritičnog oblika bolesti, nakon sedmoga dana. U tim se slučajevima preporuča primjena deksametazona, koji se u bolnici primjenjuje tijekom najviše deset dana.

Lokalna primjena sterioida također je dozvoljena, uz inhalacijsku primjenu u bolesnika s astmom.

Dozu mineralokortikoida treba povećati u svim stresnim situacijama, uključujući Covid-19. Hidrokortizon liječnik može primjenjivati kod pacijenata u stanju šoka tijekom nekoliko dana.

Primjena kortikosteroida ne preporuča se kod blagih i srednje teških oblika bolesti.

Tocilizumab liječnici koriste kad se kod oboljelih od Covida primijeti pogoršanje kliničkog stanja, progresija hipoksemije i vjerojatni sindrom prekomjernog lučenja citokina, koji obično nastupa između osmog i petnaestog dana bolesti. Njegova primjena se preporuča nakon konzultacije infektologa s internistom ili kliničkim imunologom, a precizni kriteriji za njegovu primjenu nisu definirani.

Dok je za tocilizumab, kao i za neke druge lijekove, potrebna suglasnost najmanje dva kliničara, među kojima je barem jedan infektolog, remdesivir može propisati jedan specijalist infektolog.

Za sada nema dokaza niti indikacije za istovremenu primjenu dva i više lijekova, odnosno bilo kakve njihove kombinacije, u liječenju Covida, osim u okviru kliničkog ispitivanja.

Doktor Đivo Ljubičić, pulmolog u Kliničkoj bolnici Dubrava, ukazao je na pogreške u tekstu objavljenom u srijedu na portalu Slobodne Dalmacije.

Sporni tekst prenosi savjete liječnika iz Srbije za oporavak od Covida, koji između ostalog uključuju uzimanje acetilsalicilne kiseline nekoliko mjeseci nakon preboljenja.

Budući da je izrazito zaposlen dežurstvima u Covid intenzivnoj njezi, doktor Ljubičić nije imao vremena za telefonski razgovor, ali nam je proslijedio najnovije Smjernice za liječenje oboljelih od Covida koje propisuje Hrvatski zavod za javno zdravstvo, kako bismo javnost izvijestili o trenutno dostupnim lijekovima za borbu protiv ove bolesti.

Smjernice su namijenjene kliničarima, u prvom redu infektolozima i intenzivistima, ali i drugim zdravstvenim djelatnicima koji sudjeluju u liječenju bolesnika s Covidom-19.

O primjeni se odlučuje ovisno o težini bolesti

Na samom početku navodi se kako trenutno nema dovoljno dokaza o učinkovitim i sigurnim lijekovima ili kombinaciji lijekova za liječenje Covida. To je važno osvijestiti s obzirom na postojanje niza nepouzdanih internetskih izvora koji tvrde drugačije te svojim čitateljima ili pratiteljima preporučuju raznorazne neprovjerene supstance.

U tekstu se ne navode neke od popularnih opcija, poput ivermektina ili spomenute acetilsalicilne kiseline, ali su nabrojani preporučeni lijekovi, kao i oni koje treba izbjegavati. Budući da su smjernice namijenjene medicinskim stručnjacima, sve lijekove i metode opisane u dokumentu nije potrebni prenositi. U nastavku je skraćen pregled preporučenih i odobrenih lijekova koje propisuju, i najvećim dijelom primjenjuju, zdravstveni stručnjaci.

Osnova liječenja je ublažavanje simptoma te sprječavanje komplikacija i napredovanja bolesti, uz mjere intenzivnog liječenja za teže oboljele.

Prema Smjernicama, o vrsti liječenja odlučuje kliničar na temelju procjene težine konkretnog slučaja oboljenja. 

Različiti su stupnjevi bolesti, pa se tako ona kreće od asimptomatskih infekcija, preko blagog, srednje teškog i teškog oblika, sve do kritičnog oblika koji može uključivati sepsu, zatajenje organa ili poremećaj svijesti. Teži oblik bolesti koji se manifestira kod dijela oboljelih uključuje i akutni respiratorni distres sindrom (ARDS), jednu vrstu respiratornog zatajenja koja dovodi do nakupljanja tekućine u plućima, a počinje sa zaduhom i plitkim disanjem, a zbog niske razine kisika u krvi rad srca i mozga može zakazati.

Dostupni lijekovi i preporučene doze

U Smjernicama su propisani dozvoljeni lijekovi i način njihove primjene, a neke od njih navodimo u nastavku. Njihovo korištenje ne preporuča se izvan bolnice, budući da je tijekom primjene neophodan intenzivan nadzor njihove učinkovitosti i sigurnosti, što se može postići samo u hospitaliziranih bolesnika.

Remdesivir se trenutno preporuča za bolesnike s težom pneumonijom koji imaju urednu bubrežnu funkciju. Ako su ostvareni svi preduvjeti, liječenje treba započeti što je prije moguće. Primjena nakon petnaestog dana u većini slučajeva nije opravdana. Ovaj se lijek primjenjuje u polusatnoj intravenoznoj infuziji, a kod težih se slučajeva liječenje remdesivirom provodi tijekom deset dana.

Kortikosteroidi su se pokazali potencijalno korisnima u liječenju teškog i kritičnog oblika bolesti, nakon sedmoga dana. U tim se slučajevima preporuča primjena deksametazona, koji se u bolnici primjenjuje tijekom najviše deset dana.

Lokalna primjena sterioida također je dozvoljena, uz inhalacijsku primjenu u bolesnika s astmom.

Dozu mineralokortikoida treba povećati u svim stresnim situacijama, uključujući Covid-19. Hidrokortizon liječnik može primjenjivati kod pacijenata u stanju šoka tijekom nekoliko dana.

Primjena kortikosteroida ne preporuča se kod blagih i srednje teških oblika bolesti.

Tocilizumab liječnici koriste kad se kod oboljelih od Covida primijeti pogoršanje kliničkog stanja, progresija hipoksemije i vjerojatni sindrom prekomjernog lučenja citokina, koji obično nastupa između osmog i petnaestog dana bolesti. Njegova primjena se preporuča nakon konzultacije infektologa s internistom ili kliničkim imunologom, a precizni kriteriji za njegovu primjenu nisu definirani.

Dok je za tocilizumab, kao i za neke druge lijekove, potrebna suglasnost najmanje dva kliničara, među kojima je barem jedan infektolog, remdesivir može propisati jedan specijalist infektolog.

Za sada nema dokaza niti indikacije za istovremenu primjenu dva i više lijekova, odnosno bilo kakve njihove kombinacije, u liječenju Covida, osim u okviru kliničkog ispitivanja.

Doktor Đivo Ljubičić, pulmolog u Kliničkoj bolnici Dubrava, ukazao je na pogreške u tekstu objavljenom u srijedu na portalu Slobodne Dalmacije.

Sporni tekst prenosi savjete liječnika iz Srbije za oporavak od Covida, koji između ostalog uključuju uzimanje acetilsalicilne kiseline nekoliko mjeseci nakon preboljenja.

Budući da je izrazito zaposlen dežurstvima u Covid intenzivnoj njezi, doktor Ljubičić nije imao vremena za telefonski razgovor, ali nam je proslijedio najnovije Smjernice za liječenje oboljelih od Covida koje propisuje Hrvatski zavod za javno zdravstvo, kako bismo javnost izvijestili o trenutno dostupnim lijekovima za borbu protiv ove bolesti.
