Zabilježeno je niz slučajeva gdje su osobe s narušenim imunitetom, a koje su se zarazile koronavirusom, širile SARS-CoV-2 virus znatno duže nego što je to uobičajeno. Nova studija objavljena na znanstvenom repozitoriju medRxiv, opisuje zanimljiv slučaj jedne imunokompromitirane pacijentice starije od 70 godina koja je čak 154 dana, odnosno punih pet mjeseci širila živi koronavirus, odnosno bila zarazna. To je bilo moguće jer je u njezinom tijelu tijekom tako dugog perioda bolesti virus prošao brojne mutacije.
Uzorci njezina gornjeg i donjeg dišnog trakta, koji su uzeti pet mjeseci nakon početne dijagnoze COVID-a 19, sadržavali su živi razmnožavajući virus, s vrijednostima niskog praga ciklusa lančane reakcije reverzne transkriptaze-polimeraze (RT PCR) i visokim virusnim opterećenjem.
Bolovala od limfoma
Pacijentica je u bolnicu primljena s dijagnozom COVID-a te s akutnim respiratornim distresnim sindromom (ARDS), a na odjelu intenzivne njege ostala je sve do smrti, pet mjeseci po dolasku u bolnicu. Prije zaraze COVID-a pacijentica se liječila od folikularnog limfoma te je već primala tri ciklusa kemoterapija, kao i anti-CD20 antitijelo obinituzumab, koji iscrpljuje B-stanice.
Ta je terapija završila mjesec dana prije primitka u bolnicu.
U analizi uzoraka i nakon 150 dana utvrđeno je kako je pacijentica u sebi imala soj virusa koji su definirale mutacije D614G, 3037, 14408 i 241, zajedno s brisanjem na poziciji 28881 na nukleokapsid antigenu. Ostale jedinstvene mutacije pronađene u virusu SARS-CoV-2 te pacijentice uključuju i onu koja je viđena na jugu Njemačke između 16. ožujka i 11. svibnja 2020. godine.
Znanstvenici pretpostavljaju da su se mutacije u britanskoj varijanti novog koronavirusa pojavile također kod pacijenta s narušenim imunitetom gdje je stalno dolazilo do novih upala, a virus je duže vrijeme mutirao.
Vjeruje se da se genetska raznolikost javlja unutar domaćina ako virus nije izložen selektivnom pritisku imunološkog sustava samog domaćina ili ljekovitim antitijelima.
Britanski soj došao i do Slovenije
Britanski soj koronavirusa proširio se čak u 56 europskih zemalja, uključujući i zemlje koje nas okružuju: Sloveniju, Mađarsku, Italiju i Austriju.
Kad su se pojavile prve informacije o britanskom soju ravnateljica Klinike za infektivne bolesti „Dr. Fran Mihaljević“, prof. dr. Alemka Markotić objasnila je kako više istraživanja govori da su se mutacije dogodile kod osoba čiji se imunitet ne može boriti s virusom i onda virus ima dugotrajne replikacije.
„To je primijećeno kod ljudi koji su npr. pod kemoterapijom. Kod njih se više mjeseci može detektirati virus i samim time virus može razviti promjene i mutacije koje mu omogućavaju da se duže zadrži i moguće je da su se od tih osoba raširile ove varijante virusa - ispričala je Markotić.

Zabilježeno je niz slučajeva gdje su osobe s narušenim imunitetom, a koje su se zarazile koronavirusom, širile SARS-CoV-2 virus znatno duže nego što je to uobičajeno. Nova studija objavljena na znanstvenom repozitoriju medRxiv, opisuje zanimljiv slučaj jedne imunokompromitirane pacijentice starije od 70 godina koja je čak 154 dana, odnosno punih pet mjeseci širila živi koronavirus, odnosno bila zarazna. To je bilo moguće jer je u njezinom tijelu tijekom tako dugog perioda bolesti virus prošao brojne mutacije.
Uzorci njezina gornjeg i donjeg dišnog trakta, koji su uzeti pet mjeseci nakon početne dijagnoze COVID-a 19, sadržavali su živi razmnožavajući virus, s vrijednostima niskog praga ciklusa lančane reakcije reverzne transkriptaze-polimeraze (RT PCR) i visokim virusnim opterećenjem.
Bolovala od limfoma
Pacijentica je u bolnicu primljena s dijagnozom COVID-a te s akutnim respiratornim distresnim sindromom (ARDS), a na odjelu intenzivne njege ostala je sve do smrti, pet mjeseci po dolasku u bolnicu. Prije zaraze COVID-a pacijentica se liječila od folikularnog limfoma te je već primala tri ciklusa kemoterapija, kao i anti-CD20 antitijelo obinituzumab, koji iscrpljuje B-stanice.
Ta je terapija završila mjesec dana prije primitka u bolnicu.
U analizi uzoraka i nakon 150 dana utvrđeno je kako je pacijentica u sebi imala soj virusa koji su definirale mutacije D614G, 3037, 14408 i 241, zajedno s brisanjem na poziciji 28881 na nukleokapsid antigenu. Ostale jedinstvene mutacije pronađene u virusu SARS-CoV-2 te pacijentice uključuju i onu koja je viđena na jugu Njemačke između 16. ožujka i 11. svibnja 2020. godine.
Znanstvenici pretpostavljaju da su se mutacije u britanskoj varijanti novog koronavirusa pojavile također kod pacijenta s narušenim imunitetom gdje je stalno dolazilo do novih upala, a virus je duže vrijeme mutirao.
Vjeruje se da se genetska raznolikost javlja unutar domaćina ako virus nije izložen selektivnom pritisku imunološkog sustava samog domaćina ili ljekovitim antitijelima.
Britanski soj došao i do Slovenije
Britanski soj koronavirusa proširio se čak u 56 europskih zemalja, uključujući i zemlje koje nas okružuju: Sloveniju, Mađarsku, Italiju i Austriju.
Kad su se pojavile prve informacije o britanskom soju ravnateljica Klinike za infektivne bolesti „Dr. Fran Mihaljević“, prof. dr. Alemka Markotić objasnila je kako više istraživanja govori da su se mutacije dogodile kod osoba čiji se imunitet ne može boriti s virusom i onda virus ima dugotrajne replikacije.
„To je primijećeno kod ljudi koji su npr. pod kemoterapijom. Kod njih se više mjeseci može detektirati virus i samim time virus može razviti promjene i mutacije koje mu omogućavaju da se duže zadrži i moguće je da su se od tih osoba raširile ove varijante virusa - ispričala je Markotić.
