Više od dvije trećine najčešćih nuspojava koje ljudi prijavljuju nakon primitka cjepiva protiv covida zapravo nema veze s cjepivom. To tvrde znanstvenici iz SAD-a koji su pročešljali podatke 12 kliničkih studija o cjepivima i zaključili da se kod 76 posto prijavljenih najčešćih nuspojava nakon prve doze i kod 52 posto najčešćih nuspojava nakon druge doze, zapravo radi o nocebo efektu.

Nocebo je sve ono suprotno od placebo efekta, a javlja se kad pacijenti dožive negativne nuspojave na tretmane jer to očekuju. Kod placebo efekta postižu se pozitivni rezultati tretmanom koji normalno ne bi trebao imati bilo kakav učinak, a nocebo efektom se kod pacijenta postiže obrnuti zdravstveni  rezultat zbog očekivanja najgoreg. Ovo otkriće sugerira da značajan postotak blažih nuspojava poput glavobolje i kratkoročnog umora nije uzrokovan samim sastavnicama cjepiva, već drugim faktorima za koje se zna da uzrokuju nocebo efekt, poput tjeskobe i pogrešnog pripisivanja različitih tegoba učinku cjepiva. Istraživanje tima objavljeno na u JAMA Network Openu.

Kako piše Science alert tim istraživača iz medicinskog centra „Beth Israel Deaconess“ (BIDMC) u Bostonu analizirao je reakcije 45.380 pacijenata. Od toga je 22.802 dobilo pravo cjepivo. Preostalih 22.578 pacijenata je dobilo placebo, odnosno bezopasnu supstancu bez terapeutske vrijednosti, kao što je fiziološka otopina. Nitko od pacijenata nije znao jesu li dobili cjepivo ili placebo.

Nakon prve injekcije, 46,3 posto pacijenata s cjepivom prijavilo je sistemske neželjene efekte, koji su zahvatili cijelo tijelo, najčešće glavobolju i umor. Veći postotak pacijenata, 66,7 posto, prijavio je lokalni neželjeni događaj, kao što je bol ili otok na mjestu injekcije. 

Ali zanimljivo je i što su pacijenti s placebom također iskusili neželjene događaje: njih 35,2 posto prijavilo je sistemske efekte, a 16,2 posto lokalne.

Uspoređujući odnose između dvije grupe, nocebo efekt je činio do 76 posto sistemskih neželjenih reakcija i 24 posto lokalnih nakon prve doze cjepiva. Međutim, ovaj broj je opao za drugu dozu. Manje placebo pacijenata prijavilo je neželjene događaje: 31,8 posto sistemske efekte i 11,8 lokalne. Za primatelje cjepiva, neželjeni događaji su porasli, pri čemu je 61,4 posto pacijenata prijavilo sistemske efekte, a 72,8 posto lokalne.

Dakle, čak 52 posto neželjenih reakcija nakon druge doze bilo je posljedica nocebo efekta.

Ukupno, tvrde istraživači, to znači da bi 64 posto svih neželjenih reakcija moglo biti izazvano nocebo efektom.

- Nespecifični simptomi poput glavobolje i umora, za koje smo pokazali da su posebno osjetljivi na nocebo, navedeni su među najčešćim neželjenim reakcijama nakon cijepljenja protiv covida-19 – izjavio je znanstvenik Ted J. Kaptchuk s BIDMC-a i Harvardske medicinske škole.

Naveo je i kako dokazi ukazuju na to da informacije o nuspojavama mogu dovesti do toga da ljudi pogrešno pripišu uobičajene dnevne pozadinske pojave kao rezultat cjepiva ili da izazovu anksioznost i zabrinutost zbog kojih postaju hiper osjetljivi na tjelesne osjećaje kada su u pitanju neželjeni događaji.

U prethodnim istraživanjima, tim je radio na tome da pokaže da otvorena komunikacija s pacijentima može poboljšati njihova očekivanja, posebno u vezi s placebo lijekovima koji se ponekad prepisuju kao tretman. U slučaju cijepljenja protiv covida-19, informiranje pacijenata o nocebo efektu također može pomoći u smanjenju broja neželjenih reakcija.

Iako dokazi sugeriraju da informacije o nuspojavama mogu nagnati ljude na to da uobičajene tegobe pripišu cjepivima, ili ih učiniti posebno uznemirenima u vezi s time kako se osjećaju, Kaptchuk kaže kako je potrebno ljudima davati više informacija o nuspojavama, a ne manje.

- Mnogi istraživači kažu kako bi pacijentima trebalo govoriti manje o mogućim nuspojavama kako bi se smanjila njihova tjeskoba. No ja mislim da je to pogrešno. Iskrenost je najbolji put. Medicina se zasniva na povjerenju. Naši nalazi nas navode da ukažemo na to da bi informiranje javnosti o potencijalu odgovora na nocebo moglo pomoći u smanjenju zabrinutosti oko cijepljenja protiv covida-19, što bi moglo smanjiti neodlučnost u vezi s cijepljenjem - rekao je Kaptchuk.

Više od dvije trećine najčešćih nuspojava koje ljudi prijavljuju nakon primitka cjepiva protiv covida zapravo nema veze s cjepivom. To tvrde znanstvenici iz SAD-a koji su pročešljali podatke 12 kliničkih studija o cjepivima i zaključili da se kod 76 posto prijavljenih najčešćih nuspojava nakon prve doze i kod 52 posto najčešćih nuspojava nakon druge doze, zapravo radi o nocebo efektu.

Nocebo je sve ono suprotno od placebo efekta, a javlja se kad pacijenti dožive negativne nuspojave na tretmane jer to očekuju. Kod placebo efekta postižu se pozitivni rezultati tretmanom koji normalno ne bi trebao imati bilo kakav učinak, a nocebo efektom se kod pacijenta postiže obrnuti zdravstveni  rezultat zbog očekivanja najgoreg. Ovo otkriće sugerira da značajan postotak blažih nuspojava poput glavobolje i kratkoročnog umora nije uzrokovan samim sastavnicama cjepiva, već drugim faktorima za koje se zna da uzrokuju nocebo efekt, poput tjeskobe i pogrešnog pripisivanja različitih tegoba učinku cjepiva. Istraživanje tima objavljeno na u JAMA Network Openu.

Kako piše Science alert tim istraživača iz medicinskog centra „Beth Israel Deaconess“ (BIDMC) u Bostonu analizirao je reakcije 45.380 pacijenata. Od toga je 22.802 dobilo pravo cjepivo. Preostalih 22.578 pacijenata je dobilo placebo, odnosno bezopasnu supstancu bez terapeutske vrijednosti, kao što je fiziološka otopina. Nitko od pacijenata nije znao jesu li dobili cjepivo ili placebo.

Nakon prve injekcije, 46,3 posto pacijenata s cjepivom prijavilo je sistemske neželjene efekte, koji su zahvatili cijelo tijelo, najčešće glavobolju i umor. Veći postotak pacijenata, 66,7 posto, prijavio je lokalni neželjeni događaj, kao što je bol ili otok na mjestu injekcije. 

Ali zanimljivo je i što su pacijenti s placebom također iskusili neželjene događaje: njih 35,2 posto prijavilo je sistemske efekte, a 16,2 posto lokalne.

Uspoređujući odnose između dvije grupe, nocebo efekt je činio do 76 posto sistemskih neželjenih reakcija i 24 posto lokalnih nakon prve doze cjepiva. Međutim, ovaj broj je opao za drugu dozu. Manje placebo pacijenata prijavilo je neželjene događaje: 31,8 posto sistemske efekte i 11,8 lokalne. Za primatelje cjepiva, neželjeni događaji su porasli, pri čemu je 61,4 posto pacijenata prijavilo sistemske efekte, a 72,8 posto lokalne.

Dakle, čak 52 posto neželjenih reakcija nakon druge doze bilo je posljedica nocebo efekta.

Ukupno, tvrde istraživači, to znači da bi 64 posto svih neželjenih reakcija moglo biti izazvano nocebo efektom.

- Nespecifični simptomi poput glavobolje i umora, za koje smo pokazali da su posebno osjetljivi na nocebo, navedeni su među najčešćim neželjenim reakcijama nakon cijepljenja protiv covida-19 – izjavio je znanstvenik Ted J. Kaptchuk s BIDMC-a i Harvardske medicinske škole.

Naveo je i kako dokazi ukazuju na to da informacije o nuspojavama mogu dovesti do toga da ljudi pogrešno pripišu uobičajene dnevne pozadinske pojave kao rezultat cjepiva ili da izazovu anksioznost i zabrinutost zbog kojih postaju hiper osjetljivi na tjelesne osjećaje kada su u pitanju neželjeni događaji.

U prethodnim istraživanjima, tim je radio na tome da pokaže da otvorena komunikacija s pacijentima može poboljšati njihova očekivanja, posebno u vezi s placebo lijekovima koji se ponekad prepisuju kao tretman. U slučaju cijepljenja protiv covida-19, informiranje pacijenata o nocebo efektu također može pomoći u smanjenju broja neželjenih reakcija.

Iako dokazi sugeriraju da informacije o nuspojavama mogu nagnati ljude na to da uobičajene tegobe pripišu cjepivima, ili ih učiniti posebno uznemirenima u vezi s time kako se osjećaju, Kaptchuk kaže kako je potrebno ljudima davati više informacija o nuspojavama, a ne manje.

- Mnogi istraživači kažu kako bi pacijentima trebalo govoriti manje o mogućim nuspojavama kako bi se smanjila njihova tjeskoba. No ja mislim da je to pogrešno. Iskrenost je najbolji put. Medicina se zasniva na povjerenju. Naši nalazi nas navode da ukažemo na to da bi informiranje javnosti o potencijalu odgovora na nocebo moglo pomoći u smanjenju zabrinutosti oko cijepljenja protiv covida-19, što bi moglo smanjiti neodlučnost u vezi s cijepljenjem - rekao je Kaptchuk.
